Mindful Transitions, LLC is a group clinical social work practice providing clinical mental health assessments, counseling, and clinical social work services to seniors on-site at Independent and Assisted Living residences. We assist our clients and their families in adjusting to the need for placement, adjusting to new surroundings, coping with lifestyle changes, cognitive and physical changes, and medical concerns in a gracious, supportive, and individualized manner.
Mindful Transitions LLC’s Purpose is:
To empower clients and their families to take charge of their own mental and physical wellness and health.
To inform, educate, guide, and support the client and family through the grieving process following life’s changes and losses and to assess the need for further support or resources.
To be a liaison between the client/family and the residential care facility in which the client resides and assist in ensuring quality mental health care based on the specific mental health needs of the client.
To protect and ensure optimal mental and physical health and assist in guarding against mental or physical decline leading to hospitalization or unnecessary placement.
To assist the client in reducing symptoms of depression, anxiety, isolation, and to help the client build and enhance meaningful relationships.
A physician’s order is not necessary for Mindful Transitions to come out for a client visit. The client does need to agree to voluntarily work with a Clinical Social Worker.
Medicare typically pays 80% of all sessions. Most supplemental insurance plans pay for some remaining portion of treatment rendered by a Licensed Clinical Social Worker (LCSW).
We bill Medicare and secondary insurance plans directly without inconvenience to the client, family, or residential care communities.
